Prior to your first appointment several forms need to be completed. These forms provide us with the medical information we need to provide care, and the personal information we need to contact you and your insurance company.

Two working days prior to your appointment you will be sent an email. This email offers a direct link to the patient portal to complete the Medical History form. If you are unable to complete this form you will be able to do so at the clinic. The patient registration and Consent to Treat will be completed upon your arrival. Be sure to bring your Insurance Card.

If you are a returning patient and it has been more than 3 months since your last appointment or you have injured a different body part, you will need to complete the Medical History for your current issue via the link sent in an email or in the clinic prior to your appointment.
