As part of the registration process several forms need to be reviewed and completed prior to your first appointment. These forms provide us with the medical information we need to provide care, and the personal information we need to contact you or bill your insurance company.
There are five (5) forms to be downloaded and completed. You may either fax these forms to our office at (206) 301-0601 or bring the paper work with you to the clinic on the day of your first visit.
If you are a returning patient and it has been more than 2 months since your last appointment or you have injured a different body part, you will need to re-register.

- HIPAA – this is for your review. Your signature on the Consent Form indicates that you have reviewed and been offered a copy of the HIPAA guidelines for Sound Physical Therapy.
